The first thing you will need to do to initiate your Virtual Consultation is to please complete and submit our virtual consultation form. Please make sure to include important details about yourself, such as what procedures you are considering.

Once we receive this information, our Nurse will reply as soon as possible in order to verify your information as well as answer any questions you may have on how best to connect online. A time will then be confirmed for your consultation with Dr. Bass. We look forward to seeing you!
